What sciatica is and why it hurts in your leg

“Sciatica” is not a diagnosis in itself but a description of a symptom: pain that follows the path of the sciatic nerve, from the lower back or buttock down the back or side of the thigh, and sometimes as far as the calf and foot. It may come with tingling, an electric-shock sensation, numbness or loss of strength.

The most common cause is irritation of a lumbar nerve root, often from a disc herniation or bulge, although it can also be due to narrowing of the spinal canal (stenosis) or other structures that sensitise the nerve. Not every buttock pain that travels down the leg is true sciatica: sometimes it is pain referred from a joint, a muscle or the hip, and the treatment changes accordingly.

The good news is that sciatica usually follows a favourable course: in many people it improves gradually over weeks or a few months with well-directed conservative care. Staying active within what you can tolerate, understanding what is going on and following a tailored exercise programme usually works better than prolonged rest.

At Centro Ubud, treatment is led by physiotherapists. We examine sensation, reflexes and strength, assess neural mechanics and, when it adds useful information, use ultrasound to look at structures such as the sciatic nerve as it passes through the buttock.

This treatment is for you if…

  • You have pain running from your back or buttock down your leg.
  • You notice tingling, cramping or numbness in your leg or foot.
  • It hurts more when sitting, bending over or coughing.
  • You’ve been told you have sciatica or a herniated disc and want a clear plan.

Go to A&E if you notice loss of bladder or bowel control, numbness in the genital area or between your legs, or progressive weakness in your leg. If you also have a fever, unexplained weight loss or a history of cancer, we will refer you for a medical assessment.

Frequently asked questions

Questions about sciatica

How long does sciatica last?
It varies widely. Many cases of sciatica improve within weeks or a few months, although some progress more slowly. At the consultation we estimate your case based on intensity, neurological symptoms and how long you have had it.
Is rest best for sciatica?
Prolonged bed rest doesn’t usually help. It is better to stay active within what you can tolerate, adapt the positions that irritate it most and gradually return to your activities.
What is the difference between sciatica and low back pain?
Low back pain is centred on the lower back. In sciatica the main pain runs down the leg, often with nerve symptoms such as tingling or numbness.
Do I need an MRI scan?
Not always. It is recommended when there are red flags, significant weakness, or no improvement after a reasonable period of treatment. If we think it is necessary, we will point you towards a medical assessment.
How many physiotherapy sessions will I need?
It depends on how irritable the nerve is and how you respond. Usually a limited number of clinic sessions is combined with exercise at home, reviewing the plan as you progress.
Is the treatment painful?
We aim not to increase your symptoms. Some exercises may cause mild, short-lived discomfort, but we adjust the dose so your leg pain doesn’t get worse.
How much does sciatica treatment cost in Torremolinos?
It depends on your case, how long you have had it and the techniques recommended. At the free consultation we give you a fixed quote with no obligation.
